Medical Care is provided free of charge for patients between the ages of 19 and 64 without health insurance and with income up to 200 percent of the federal poverty level.
Clients are seen by appointment after being screened for eligibility.

Eligibility:

Resident of Montgomery, Autauga, or Elmore Counties
Between the ages of 19 and 64
Social Security number and proof of that number
NOT have Medicare, Medicaid, VA medical benefits, or private insurance

For the eligibility screening, clients are asked to provide proof of household income (such as Social Security, disability, unemployment, child support, food stamps, salary, etc.) and picture identification with Social Security number.
Medical Outreach Ministries is a non-profit organization that exists to serve people in Montgomery, Autauga, and Elmore Counties who cannot afford routine medical care for themselves.
While the poorest of the poor may be eligible for Medicaid or other public assistance, there are many uninsured persons who fall through the cracks of the healthcare system.
Their only access to a physician for even the most common illnesses is often through the local emergency rooms.

As a witness for Jesus Christ, Medical Outreach Ministries exists to minister to the physical, emotional, and spiritual needs of the medically underserved by providing free healthcare services.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
