CCC treats a variety of ADULT (18 years - 64 years) conditions, such as Hypertension, COPD, Asthma, Emphysema, Diabetes as well as Bronchitis, mild sprains and mild dermatologic (skin) conditions. We do not keep narcotics on the premises nor will we write prescriptions for any narcotic medications.

For those patients with chronic conditions: we will assist in completing the applications for Free Medications offered by Pharmaceutical houses.
Care is provided to those patients who do NOT have any type of health insurance with limited income. We ask patients to bring a photo id, proof of income for either the last year and or the last few months as patients will need to qualify financially for services provided at the clinic.

Our Primary Health Clinic provides screenings for adults
ages 18 through 64 with chronic health conditions - arthritis, asthma, cancer, COPD, diabetes, emphysema, epilepsy, depression, heart diesase, hypertension, and obesity. When possible, physicians may refer patients to specialists for extended care. We will do our best to provide specialist care if available. No narcotics are on the premises nor will we write prescriptions for any narcotic medications.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
