The Wellness Clinic offers integrated primary and behavioral healthcare and addresses a broad range of physical health needs including management of high blood pressure and diabetes. Educational groups on topics such as smoking cessation, diabetes management, and fitness and health, provide wellness support. An onsite therapist is also available.  The Wellness Clinic serves the general adult population as well as individuals with mental illness, ages 18-64 who do not have insurance, or are insured by Medicaid or the Polk Health Care Plan.

Hours: Mondays through Fridays 8am - 5pm.
