I don't have insurance and could not afford see primary doctor for thyroid lab check up and refill and I had back injury needing mri for neck and lower back ,I had x ray done but told me I have to see primary doctor I have not able to afford. I had back injury for my lower back as refuse to proceed on the upper back.

I have 2 teeth broke off in the gum in the back and cavities in most of my teeth now teeth now, my mother and father weren't so kind and damaged my teeth as a teenager.

job related injury attributes to majority of issues, currently and have been unemployed since june15,2018

I had back injury and going to voctional rehabition for review what kind of restriction lifting before getting job but I cannot go ahead until upper back is check out first. please I need some assistance and move forward living.
