For an appointment, please call 904-394-8069. While walk-ins are accepted, appointments are preferred.
   Primary Care: Monday - Thursday 8:30AM - 5:00 PM; Friday 8:30AM - 12PM
   Dental: Monday: Thursday 8:00 AM - 4:00 PM; Friday 8:00 AM - 12. Closed daily from 12:00 - 1:00PM
   Behavioral Health: Monday - Friday 8:00 - 4:30. Closed daily from 12:00 - 1:30PM

Funded through the Health Resources and Services Administration (an agency of the US Department of Health and Human Services), Sulzbacher offers comprehensive healthcare to anyone in the community in need.  Sulzbachers Health Centers are classified as Federally Qualified Health Centers (FQHCs) and as such, are designed as safety net providers that serve medically underserved areas and populations. Services are provided on a sliding fee scale based on the patients ability to pay, and all insurances are accepted.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
