We are committed to serving the insured and uninsured.
A sliding fee scale is available to make treatment affordable to those without insurance. Fees are calculated based on household size and documented annual income level.


Midtown Health Center (formerly Norfolk Community Health Care Clinic) was established in October 1999 in Norfolk, NE. The Madison Clinic was added in 2008. We are a non-profit 501 c3 Federally Qualified Health Care Clinic (FQHC) serving the greater Norfolk and Madison area. The board of directors consists of 12 volunteers from both communities with expertise in various areas. They govern the clinic's operations and seek funding to provide quality affordable health care to residents of Northeast Nebraska. Currently the two clinics employ a staff of 40 plus between the medical, dental and specialty services. You're not just another patient. We truly care about you and your family. We offer Quality Care Close to Home. New Patients are welcome. We look forward to serving you.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
