Requirements:
Eligibility is based on proof of
household size and income. All amounts due
from the patient whether insured or uninsured
can be discounted based on provided proof of
income. We offer pay-in-full discounts and
deferred billing.

Fees:
Uninsured patients with income less than 200% of the federal poverty scale will pay $40 for the office visit and a minimum of $17 for lab work. If unable to pay that amount, patient will be charged a minimum of $20 for services. If insured with income less than 200% of the federal poverty scale, patient will pay a $7 co-pay. Traditional Medicare patients with income less than 200% of the federal poverty scale will not pay a deductible and co-insurance will be reduced to $15.
