Free Clinic.

Reach Out is a 501(c)3 volunteer healthcare organization.

Reach Out provide access to health care services for the underserved/uninsured population in Montgomery County, without restraints, utilizing the professional skills of volunteer physicians, nurse,s and other health care professionals.

Eligibility:

  • Reach Out patients live in Montgomery County, Ohio, and are not homeless.
  • Reach Out patients have no type of health insurance or assistance.
  • Reach Out patients have a monthly income less than 200 percent of the Federal Poverty Guidelines as listed below.

Identification is required at clinic registration.

A photo ID is preferred (driver's license, state ID).

Eligibility for patients living outside of Montgomery County will be determined on a case-by-case basis.


Reach Out provides a safety net clinic located minutes from downtown Dayton.

The clinic offers an alternative to emergency visits for minor illnesses and chronic conditions that need immediate attention.

Clinic visits are at no cost to the patient. Ancillary services such as lab and x-ray are limited based on current contracts.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
