The Centers are comprehensive, multi-specialty primary care facilities offering affordable, accessible, high-quality medical.

Dental, General Adult Care, Pediatrics, Laboratory, X-Ray, Diabetes and Hypertension Counseling, Insurance Counseling, Pharmacy, WIC, and Behavioral Health


Financial assistance and charity care are available to patients that apply and qualify. If you need help filling out the application, contact one of the financial counselors at 423-778-5150. In addition to helping with charity care, financial counselors are here to help you figure out if you can get TennCare or Medicaid, and how to pay for your care. If you don't qualify for financial assistance, they allow some patients to make payments on a monthly basis.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
