Fowler Memorial Free Dental Clinic is located on the first floor of the Government Services Building of the Pleasant View Complex, just East of Monroe, Wisconsin on Highway 81.
Clinic hours:
8 a.m. to 12 noon on Fridays.
The clinic is not seeing adult patients at this point in time. Only children in Green County, WI between the ages of 3-12 are seen by appointment.

To provide oral care and education to underinsured and uninsured children living in Green County Wisconsin.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
