Proof of Household Income
Insurance Card-Participating Provider for Blue Cross and Blue Shield, Medicaid and Medicare.
Some services require Guardianship paperwork for minors if parent is not present.

The Emergency Room is no longer your only option for conditions such as fever, infection, sore throat, cough, high blood pressure and diabetes. If you wish to escape the long waits and do not have a life-threatening injury, consider visiting the Village Health Center. Once your immediate need is met, we can provide you with a professional referral, linking you to services for the management of chronic illnesses. We will work with you to help you find a primary care doctor so you do not have to rely on the emergency room for such management.
Services are based on a sliding fee scale, with a minimum charge of $15.

Exception: Last Thursday of each month, our clinic may be closed in the afternoon for staff training.
The Village Health Center services are by walk-in only, no appointment is needed.
