HLH was founded as non-profit in 1986 by a group of Native Hawaiian physicians and concerned citizens.
In 1991 the organization gained a professional status under the Native Hawaiian Health Care Improvement Act.
The organization is trying to improve the health status of Native Hawaiians, which have the highest rates for diabetes, obesity, cardio-vascular disease, and poor dental health.
At the basic core of these problems is proper diet, nutrition, and lack of exercise.
In 2001 the organization expanded and became the first Federally Qualified Health Center for the county of Kaua`i serving all people living on the island.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
