5 medical clinic days:
Monday through Friday, 8 a.m. to 12 noon and 1 p.m. to 4 p.m.
Tuesday night, 5 p.m. to 8 p.m.
Dental extraction clinic 2 nights per month, eye disease exams for diabetics at a local ophthalmologist's office, a once a month hearing clinic, diabetic supplies and referrals to specialists in Cullman and Birmingham.
Resident of Cullman County (acceptable proof is a utility bill from where you live),
have two forms of identification (such as driver's license, Social Security card or birth certificate) and proof of income (last year's federal tax return.
If you did not file a federal return, you must provide a copy of your Medicaid denial letter).
You must also not be covered by Medicare, Medicaid, veteran's or private insurance.

Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:
Symptoms such as fever, cough or shortness of breath, fatigue, headache, muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose.
Contact with someone with confirmed COVID-19 within 14 days of onset of systems
Low-grade fever (approx 100 degrees Fahrenheit for adults)
Mild, dry cough
Mild body aches
Fever above 100.4 F
Temporary shortness of breath when you exert yourself
Exhaustion, need to stay in bed
Constant trouble breathing
Persistent chest pain or pressure
Trouble staying awake
Blue lips or face
