5 medical clinic days:
Monday through Friday, 8 a.m. to 12 noon and 1 p.m. to 4 p.m.Â
Tuesday night, 5 p.m. to 8 p.m.Â
Dental extraction clinic 2 nights per month, eye disease exams for diabetics at a local ophthalmologist's office, a once a month hearing clinic, diabetic supplies and referrals to specialists in Cullman and Birmingham.
Resident of Cullman County (acceptable proof is a utility bill from where you live),
have two forms of identification (such as driver's license, Social Security card or birth certificate) and proof of income (last year's federal tax return.Â
If you did not file a federal return, you must provide a copy of your Medicaid denial letter).Â
You must also not be covered by Medicare, Medicaid, veteran's or private insurance.
