Contra Costa Health Services (CCHS) is a comprehensive county health system that cares for and improves the health of all people in Contra Costa with special attention to those who are most vulnerable to health problems.
Hours of Operation
Mon., Wed., Fri., Sat.: 7:45 a.m. - 11:45 a.m., 12:45 p.m. - 4:45 p.m.
Tuesday & Thursday: 7:45 a.m. - 11:45 a.m., 12:45 p.m. - 6:15 p.m.
*Center closed during the lunch hour and on all major holidays.
Monday - Friday: 12 - 8:15 p.m.
Saturday: 8 a.m. - 4:15 p.m.
*Behavioral Health closed on all major holidays.
Primary care, short-notice/same-day appointments, Family Medicine, Pediatrics, Adult Medicine, Behavioral Health and Group Classes.
Contra Costa Health Services has instituted a Charity Care Program and a Discount Payment Program to enable qualifying uninsured and underinsured individuals to obtain free or discounted medical care.
Who Is Eligible for Charity Care?
Residents of Contra Costa who:
have income that does not exceed 150% of the federal income guidelines.
have assets with values no greater than $2,000 per individual or $3,000 per family, excluding retirement accounts.
have real property with values not exceeding $6,000.
are not eligible for Medi-Cal or Healthy Families.
are not members of HMO or PPO health insurance plans.
Citizenship or legal residency is not required.
No cost to eligible persons.
Who Is Eligible for Discounts?
Eligible individuals do not have to be residents of Contra Costa.
Individuals whose income does not exceed 350% of the federal income guidelines.
Individuals must be uninsured or underinsured to the extent they have out of pocket medical expenses in the prior 12 months that exceed 10% of the family income.
Assets and real property excluded from eligibility determination.
Citizenship or legal residency is not required.
Eligible individuals receive a 35% discount on their medical bills.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:
- Symptoms such as fever, cough or shortness of breath, fatigue, headache
- Symptoms such as muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose.
- Contact with someone with confirmed COVID-19 within 14 days of onset of systems
Mild Covid Symptoms
- Low-grade fever (approx 100 degrees Fahrenheit for adults)
- Nasal congestion
- Runny nose
- Mild, dry cough
- Mild body aches
Moderate Covid Symptoms
- Fever above 100.4 F
- Persistent cough
- Temporary shortness of breath when you exert yourself
- Exhaustion, need to stay in bed
Severe Covid Symptoms
- Constant trouble breathing
- Persistent chest pain or pressure
- Trouble staying awake
- Blue lips or face
