This clinic operates under a SLIDING SCALE model. This means that it MAY NOT be free depending on your income. Davis Street Primary Care Clinic provides multi-lingual, community-responsive, and culturally-appropriate medical, dental, and behavioral health care services at a single location! In combination with Davis Street, we aim to provide a holistic safety-net approach encompassing many services ranging from food bank and emergency food services, clothing, children's development and care, and housing/employment/transportation assistance. We have multi-lingual translation capability.
Services and Hours
Medical services: Monday, Tuesday, Wednesday, Friday-8:30 am to 5:00 pm.
Thursday-8:30 am to 2:30 pm, Walk-ins and Appointments welcome.
Dental services: Monday, Wednesday, Friday, by appointment only.
Behavioral Health services: Monday thru Friday, by appointment only.
The following insurances are accepted: Medi-Cal, Medicare, Anthem Blue Cross, Blue Shield of CA, and Denti-Cal.
If your gross household income is <200% of the Federal Poverty Guidelines Level, we provide a Sliding Fee Scale Discount, dependent upon your Income level and Household Size.
What to bring
Upon your first visit, please bring the following:
Current valid Photo ID (example Drivers License)
Current proof of income (example 1-months worth of pay stubs)
Current Medi-Cal insurance card (if applicable)
If you do not have these items with you, you will be asked to re-schedule and come back.

These numbers above represent 100% of the Federal Poverty Rate. In order to get legal aid from some offices, they use a sliding fee scale. When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:
Symptoms such as fever, cough or shortness of breath, fatigue, headache, muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose.
Contact with someone with confirmed COVID-19 within 14 days of onset of systems
Low-grade fever (approx 100 degrees Fahrenheit for adults)
Mild, dry cough
Mild body aches

Temporary shortness of breath when you exert yourself
Exhaustion, need to stay in bed
Constant trouble breathing
