The Community Health Access Coalition (CHAC) is a non-profit, volunteer organization coordinating access to healthcare for uninsured residents of Chippewa, Luce, and Mackinac Counties in the Eastern Upper Peninsula of Michigan (EUP).
CHAC Eligibility Requirements are clear and the Enrollment process is simple.
Checklist to determine if you are eligible for CHAC healthcare services.
Are between the ages of 19 and 64
Are a resident of Chippewa, Luce or Mackinac County
Do not have health insurance (i.e., Blue Cross)
Do not have Medicare or Medicaid (except Medicaid Spend-down/Deductible)
Contact the office and they will help you determine if you are eligible for CHAC healthcare services. If you are eligible, they will schedule an appointment for you to visit the office to enroll.
