Community/Migrant Health Center (C/MHC) and Colorado Indigent Care Program (CICP) are programs with specific income guidelines. Eligibility and co-payments are based on household size and inÂcome. An Eligibility Specialist will assist you with an application.
Information needed to apply includes:
One month of income verification from the prior month. This would include copies of paycheck stubs or income verification from your employer. You will receive assistance with eligibility even if you are unemployed or have no income.
Current driver's license or Colorado ID.
At the time of enrollment, patients who meet the guidelines will receive information on the specific services they are eligible for. Sliding Fee Program cards are issued at the time you are determined eligible. This card must be presented to the cashier/receptionist at the time of each visit. Sliding fee programs are not health insurance. Co-payments are expected each time you have an appointment at the clinic or for each prescription drug prescribed by your health care provider.
