The Community Outreach Clinic provides non-emergency primary care for adults in Quincy and the surrounding area at no cost to the patient. If more specialized care is needed, the clinic physician or nurse practioner will make referrals to other health care facilities. When appropriate, the clinic staff also will assist individuals by referring them toward available social service resources.
Eligible care recipients are adults, 18 and over, not qualifying for health care through Public Aid, Medicare or other health insurance plans. Eligible care recipients must complete an uncompensated care application provided by the Community Outreach Clinic.
Primary care includes treatment for new and ongoing health problems like diabetes, high blood pressure, arthritis, heart disease, acute and chronic respiratory and digestive problems.
Prescription medications may be supplied through the Catholic Charities Med Assist Program. Community Outreach Clinic patients must apply with Catholic Charities to become part of the program. More information is available by calling Catholic Charities at 222-0958.
