COVID-19 (Coronavirus) Information for Patients
Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:
Symptoms such as fever, cough or shortness of breath, fatigue, headache, muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose.
Contact with someone with confirmed COVID-19 within 14 days of onset of systems
This clinic operates under a SLIDING SCALE model. This means that it MAY NOT be free depending on your income. You will be required to prove financial need in order to receive free services or services at a reduced cost. In order to get more information on this clinic, click on the icons below. IMPORTANT: This clinic is ONLY open to agricultural workers and their families. If you need to see a doctor or dentist, please call for an appointment so we may schedule you for a day and time when the doctor or dentist is in.

For patients who do not have health insurance, we offer a sliding fee scale based on family size and income. We also accept Medicaid for dental services. Unfortunately, CHP Aurora is not enrolled as a Medicaid Managed Care provider.
FOR PATIENTS WITHOUT INSURANCE:
Sliding Fee Scale Discounts for Patients without Insurance
Patients without health insurance may be eligible to receive services at a reduced fee. Eligibility for the Sliding Fee Scale discount is based on family size and income. If you are eligible for a sliding fee discount, you will be responsible for your part of the charges after the discount is applied at the time of each service.
Please bring verification of all sources of income for all adult members in your family. Our staff will determine the discount for which you are eligible. Verification of Income includes any of the following that pertain to your or your family's situation:
- Employer Statement of Earnings
- W-2 form(s) for last year's income
- Last year's tax return
- Check stubs from current employer(s)
- A letter from your employer verifying your employment and pay rate
- Unemployment, disability, alimony, pension, or Social Security retirement benefits
Patients who are not Eligible for the Sliding Fee Scale Discount
Patients without insurance, who are not eligible for the Sliding Fee Scale discount, will be responsible for paying the full charges at the time of service.
