For five years, the Community Health Center of Jackson County, located in Seymour, has provided high quality, client-driven health care that is affordable and accessible.
Board certified doctors, dentist, nurse practitioners, nurses, dental hygienists, social workers and staff serve many patients a year. We are committed to our patients, their families, and this community, offering a medical home and total family care for 17,040 of our patients.
Our focus is the underserved, uninsured, and under-insured in Seymour, Jackson County and surrounding areas. We accept Medicaid, Medicare, and private insurance, and offer services on a sliding fee scale. For the convenience of our clients, we also offer a transportation voucher.

- Fever above 100.4 F
- Persistent cough
- Temporary shortness of breath when you exert yourself
- Exhaustion, need to stay in bed
Severe Covid Symptoms
- Constant trouble breathing
- Persistent chest pain or pressure
- Trouble staying awake
- Blue lips or face
