The Clinic is located at 205 South Commerce Cove. Take Hwy. 7 South towards Water Valley. We are 2.5 miles past the intersection of Belk Blvd. and Hwy. 7. Turn left onto South Commerce Cove and follow it around to the back. The sign in front says OMMC

What services are provided at the Clinic?Primary care for adults with chronic illness.Limited laboratory testing ordered by OMMC doctorsPrescription medicines (no controlled drugs)Health education counseling, basic x-ray When is the Clinic Open?Monday - Thursday 9:00 a.m. - 4:00 p.m.Patients are seen by physicians Tuesdays and Wednesdays by appointment. Other days are used for screening, referrals, pharmacy services, monitoring and patient education. What do I need to qualify for services?Call for an appointment when you have the following documents.Proof that you live in Lafayette or Yalobusha County, Mississippi ( ex. Utility Bill)Two most recent pay stubs from your place of employmentPhoto IDYour Social Security cardMost recent Federal Tax Return & W-2's
