Individual, group, and family therapy

Lifestyle change management

Dental:

Adult and child cleanings
Dental sealants
Wellness and STD screenings
Fluoride varnish treatments
Oral cancer screening
Screening for cavities and prevention education
Referrals to local dentists for exams, X-rays and treatment
Referrals to oral surgeons or periodontists as needed


Prevention / Wellness:

Nutrition education
Smoking cessation
Weight loss
Sleep management
Sports physicals
Diabetes Empowerment and Education Program (DEEP) classes
Stress management
Exercise plans


Additional Services:

English/Spanish written information & bilingual staff available upon request
Screening/referral/help with health insurance, Medicaid, food, housing, school, legal, job services, etc.
Limited transportation to our clinics
Mobile clinic


Eligibility:

- Agricultural workers and immediate families.

- The majority (51%) of their labor to some form of agricultural work.


Staff members are always available to help determine eligibility.


Payment:

- Based on patient income, sliding scale discounts are available.

- Services are provided to those eligible regardless of ability to pay.


Provide assistance with applications for Medicaid, Medicare, private insurance and other social service resources.
