The Sliding Fee Scale Program is available to uninsured residents of New York City and Long Island who are ineligible for public health insurance. This program is for assistance paying for multiple treatments, services or ongoing care. Applicants must also meet income requirements.
The income restrictions for program eligibility are set based on the federal poverty level and a patient's household size.

2018-01-20 21:49:42I am looking for medical care for my husband who lost his insurance and has a shunt in his head which needs to be monitored. He ran out of his medications and since he has no insurance, we cannot get them. He has applied for Medicare but he only got Part A. He is not eligible to get Part B until July 1, 2018.

2018-01-20 21:49:42Before he had insurance, he was seen by his neurologist at North Shore and his primary doctor. He has not been seen by his doctors since he lost his insurance last year. We are not getting any income in for the last two years and have primarily been living off of our savings. My husband applied for disability social security and still waiting to hear. He was denied unemployment.

2018-01-20 21:49:42My husband lost his job two years ago and had two brain surgeries. One to put a shunt in the brain and then a month later he developed a hematoma as a result of the shunt running too fast pumping the fluid out of his brain. Our son passed away suddenly two months ago and this is also affecting my husband. He very depressed and declining cognitively as well. He needs to be seen by a doctor.
