Tuesday Hours
8:45 am - 6:00 pm

Adult patient eligibility requirements - 18-64 yrs.
To be eligible, you must:.
    Be a resident of mid or upper Pinellas County (North of Park Blvd)
    Be a US citizen or Permanent Resident of the US
    Meet low-income financial guidelines
    Not have any insurance, including Medicare/Medicaid, County Social Services, VA Benefits

Pediatric patient eligibility requirements - infants to 17 years of age.
To be eligible, you must:
    Be a resident of mid or upper Pinellas County (parents must provide documentation of address)
    Be a US citizen or lawful permanent resident
    Meet low-income financial guidelines
    Not have any insurance, including Medicaid, County Social Services, KidCare

Patients have access to a full range of medical services that are
provided on-site within their primary care setting. Other services,
including diagnostic procedures, are provided by community partners.
Comprehensive care includes:
    EKGs, labs, and x-rays
    Hospital procedures (as warranted)
    Pap smears and mammograms
    Care, nutrition and education
    Acute and chronic medications
    Patient Assistance Program enrollment
    Diagnostic eye exams (when available)
    Diabetic eye exams
    One-on-one counseling and group classes
    Sick visits
    Back to school physicals
    Referral only from Clinic practitioner

In addition: Physical Therapy, Ear, Nose and Throat Program, Patient Education Program & Coumadin Clinic.
