Our Sliding Fee Program provides reduced prices on office visits and all other clinic services to individuals and families who qualify. Prairie Community Health offers the Sliding Fee Program at all of our clinic locations.

The Sliding Fee Program is based on family income and may reduce your bill from 20 to 80 percent or more. You may qualify with or without insurance and you may qualify even though you have Medicare or Medicaid. To see if you are eligible, refer to the table below. Find your family size and if your family's annual income is less than the amount listed, you qualify!
