Travel to countries with coronavirus alerts within 14 days of onset of symptoms or
Contact with someone with confirmed COVID-19 within 14 days of onset of systems

Requirements for care are as follows:
Â Â Â Not eligible for Medicaid, Medicare, Veterans, or other insurance
Â Â Â Must be between 18 and 64 years of age
Â Â Â Must register at the clinic
Â Â Â Must be a Franklin County Resident
Â Â Â Income at or below 200% of the federal poverty guidelines
Â Â Â Â Â Â Â Single Person - up to $22,980 a year
Â Â Â Â Â Â Â Couple - up to $31,020 a year
Â Â Â Â Â Â Â Family of 4 - up to $47,100 a year - higher income may be had with more family members
Â Â Â Limited assets
Bring the following information to your registration appointment:

- Proof of residency in Franklin County
- Â Â Â Proof of income for the entire family (last 1040 Federal Income Tax Return)
- Â Â Â 4 most recent pay stubs
- Â Â Â Unemployment benefits letter
- Â Â Â Disability check or Letter of Benefits
- Â Â Â Social Security check or Letter of Benefits
- Â Â Â Food Stamp letter (monthly amount)
- Â Â Â Real Estate tax paper
- Â Â Â Bank statements (checking, savings, CD's, stocks, bonds, etc.)
YOU MUST REGISTER AHEAD OF TIME TO SEE A PHYSICIAN.
Call the clinic at 540-489-7500 between 8:00 A.M. and 4:00 P.M. Monday through Thursday for an appointment to register. You will be asked to bring the following items that apply to you. If you bring all required information to register, approval for services can be provided the same day as registered.
