While services are not free, eligible patients can receive reduced health care costs based on income and family size.

This clinics services include:

• Primary Care - immunizations, physicals, chronic care management, labs, urgent care services
• Behavioral Health - counseling, substance use disorder treatment and peer support services

Clinic Hours:

• Monday - Thursday 8:30am to 5:00pm (closed 12:00pm to 1:00pm)
