A Community Clinic, Inc. assists the uninsured population with access to high quality health care through a network of volunteer physicians and services provided by four area hospitals. Patients are treated at the clinic and referred to the area hospitals if further testing is needed. The four area hospitals, Evangelical, Geisinger, Shamokin and Sunbury, have donated a limited number of radiology and laboratory services for clinic patients.
ELIGIBILITY REQUIREMENTSMust live in Union, Snyder, Montour or Northumberland County.Must be younger than 65 years of age.Must have NO form of health insurance. (We do not accept CHIP Medicaid, Medicare, GHP, Blue Cross/Blue Shield, Veterans Benefits or any other form of insurance.)Must bring in most recent 1040 Federal Tax Return or Benefit Award Letter for most recent filing year for everyone in the household.Photo IDApply for Medical Assistance within 45 days of your first visit.Financial eligibility is determined by a percentage of the Federal Poverty Level.
