Hours: Mondays through Fridays 8:30am to 4pm.
Patients are seen by appointment only and must meet eligibility requirements. All services are available in English and Spanish. If you have questions about services or eligibility, please call 817-632-6020. The clinic is located at 3500 Noble Avenue Fort Worth, Texas 76111. We serve people who are 18 years or older, without health insurance, and with an income not greater than 250% of National Poverty Level. Se habla español.

*We do not accept or bill insurance. If you have Market Place coverage, you must provide proof of your high co-pay and inability to pay. Donations are not accepted in return for clinic services.

Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:
Symptoms such as fever, cough or shortness of breath, fatigue, headache, muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose.
Contact with someone with confirmed COVID-19 within 14 days of onset of systems
Low-grade fever (approx 100 degrees Fahrenheit for adults)
Mild, dry cough
Mild body aches
Fever above 100.4 F
Temporary shortness of breath when you exert yourself
Exhaustion, need to stay in bed
Constant trouble breathing
Persistent chest pain or pressure
Trouble staying awake
Blue lips or face
