Federally Qualified Health Center
DOH-Sarasota partners with Community Health Centers of Sarasota County, Inc. a Federally Qualified Health Center (FQHC), to provide primary health care services at our locations. No one is refused health care. However, everyone must pay their fair share based on income and family size. Proof of Sarasota County residency is required for certain facilities that handle referrals from primary care providers.

Since this is a sliding fee scale clinic, we have provided the Federal Poverty Guidelines below. Visit the North Port Community Health Center - Sarasota County Health Department website listed above to see what the level is needed for free care.

Federal Poverty Guidelines for 2021

Persons In Family Household Poverty Guideline Salary per year
1 $12,760
2 $17,240
3 $21,720
4 $26,200
5 $30,680
6 $35,160
7 $39,640
8 $44,120

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income. When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.

Best free or low-cost clinic I've ever received care from. You will be there a long time, and this is because they are very thorough! Be prepared to wait excruciatingly long if you were too lazy to make an appointment, but it will be worth it. You won't be sorry that you put your faith in them...c'mon, appreciate! I have the utmost gratitude for this clinic, and I dig my doc too! (Dr. Malliot)
