This clinic has provided great health care for Schenectady County and the surrounding areas for over 35 years. They have grown from a small health center to a complete health network offering reliable, high-quality services that you and your loved ones need to get and stay healthy.  Good health is their highest priority with their caring, experienced, and board certified providers that will work with you in their comfortable, confidential family practice setting to make sure that all of your health and wellness needs are met. Services provided are from prenatal care to pediatrics, from coughs and colds to chronic disease care.

Wednesday  8 am to 5:30 pm
Thursday  8 am to 5:30 pm
Friday  8 am to 5:30 pm
Saturday  8:30 am to 1 pm

Hometown Health Center patients will be seen by appointment with their primary care provider by calling 370-1441 during regularly scheduled business hours. Patient appointments are scheduled by members of the registration staff or Call Center employees. Health center patients are encouraged to schedule appointments with a provider, but the Center also offers emergency urgent care and same-day walk-in appointments daily to increase access to care in the community.

When they use a sliding fee scale, the 100% rate can be different than 100%. Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:

  • Symptoms such as muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose.
  • Contact with someone with confirmed COVID-19 within 14 days of onset of systems

Mild Covid Symptoms

  • Low-grade fever (approx 100 degrees Fahrenheit for adults)

Moderate Covid Symptoms

  • Fever above 100.4 F
  • Persistent cough
  • Temporary shortness of breath when you exert yourself
  • Exhaustion, need to stay in bed

Severe Covid Symptoms

  • Constant trouble breathing
  • Persistent chest pain or pressure
  • Confusion
  • Trouble staying awake
  • Blue lips or face
