Perry Family Free Clinic increases access to healthcare services for uninsured and under-insured Black men, and all people who are impacted by health disparities in Dane County.

Services include:

• Primary Care
• Mental Health
• Diabetic Foot Care
• Physical Therapy
  

To be eligible for services, the patient must meet the following criteria:

• Live in Madison or Dane County
• Have no insurance, including no Medicaid, no Medicare, and no veteran benefits
• Have monthly income that is at or below 200% of the federal poverty level

Clinic Hours:

• Thursdays 1:00pm to 5:00pm
