General Requirements:
Patients must reside or work in Coffee, Franklin or Moore counties
Must provide proof of residency (e.g. current utility or phone bill showing in-county residence) and most recent pay stubs, tax return (if available) and mail showing residency within any of the 3 counties served
Patient (or at least one person within household) must be working a minimum of 20 hours per week
Cannot be eligible for TennCare and not have any other health insurance coverage
Household income must fall within 200% of the Federal Poverty Guidelines
Fees: This is a free clinic for the working uninsured.
