Payment for Services
Payment is requested at the time of service and may be made by cash, check, money order, Visa, or Mastercard. TennCare coverage is accepted through Blue Care, John Deere, PHP and TennCare Select. Medicare and most private insurance are accepted. Co-payments are the patient's responsibility. You may be eligible for reduced fees for services, but federal guidelines require patients to provide proof of income and address at the time of service. You may bring paycheck stubs, receipts, or current utility bills showing your name and address for proof.

Our board-certified or board-eligible professional providers include pediatricians, internal medicine specialists, an obstetrics and gynecology physician who is a Fellow in the American College of Obstetrics and Gynecology, nurse practitioners, nurse mid-wives, and dentists.
The Health Centers also provide cutting-edge disease management and wellness promotion services to our patients. Access to emergency care hospitalization or other specialized services is provided through our affiliation with the Erlanger Health System. We provide health care regardless of the patient's ability to pay.

These numbers above represent 100% of the Federal Poverty Rate. In order to get legal aid from some offices, they use a sliding fee scale. When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
