Services at this location include:

  • Medical: primary care, well-child visits, developmental screening, immunizations, school/sports physicals, and pediatric specialty care.
  • Dental: cleanings, preventive care, restorative care, fillings and crowns, root canals, tooth removal, infections, retainers, braces, scaling, root planning, dentures and bridges.

CHN accepts most private insurances, Medicaid, Medicare, and CHIP. Uninsured individuals may be eligible for discounted self-pay services based on their family size and household income.

CHN does not turn away anyone away due to an inability to pay.

    I am filling this out for my aunt, whim is 54 yo and has been taking care of 2 young children for several years now and putting her health on hold due to no insurance.
