    Unemployment Benefits, Social Security, or other public assistance.
    Names, birthdates and social security numbers of each person in your household.
    Most recent income tax return (W2/1040) or one month of pay stubs for every working adult in the home.



Our integrated network model affords patients convenient and affordable access to high quality Dental Services to those who are uninsured or underinsured.

Our affordable, high quality dental services are the only available option for many South Plains and Texas Panhandle residents who are without dental insurance and would otherwise be unable to afford the cost of dental care. Services include cleanings, x-rays and preventive care, fillings, root canals, extractions, endodontic, prosthedontics, and dentures.
