Glendale Health Center is a leader in the implementation of the medical home model to increase access to care and enhance the patient experience. Our registered nurses serve as care managers for patients to ensure that the right care is delivered at the right time. We provide primary care for adults and children, and also offer support through social work, nutrition counseling and laboratory services.

At Glendale Health Center, we are continually looking for ways to better serve our patients. We have dramatically reduced the number of patients missing appointments by calling our patients before their appointments to confirm the day and time, as well as encouraging them to bring any medications or other necessary information. As a result, most of our patients now make their medical appointments, which is important for patient health and well-being.
Our nurses make outreach calls to remind patients of their appointments, which provide patients an opportunity to ask questions about their care and receive reminders about completing all lab tests prior to the doctor visit. Through these personalized contacts, we strive to make the experience of health care easier on the patient and improve both clinical efficiency and health outcomes.
