About Crossroads Medical Mission

The purpose of Crossroads Medical Mission is to provide free medical care to the uninsured, low income citizens living in the county, which recently has expanded to include Walton County.

To be eligible to participate with Crossroads Center you must be:

A resident of Okaloosa or Walton County

A patient without health insurance

Household income does not exceed 200% of Federal Poverty Guidelines

A U.S. Citizen or legal resident

An Adult (18 years of age or older)

Administrative duties:

Tuesday - Thursday 9:00am to 1:00 pm.

Since all services are provided by volunteers, clinic hours are limited and only available by appointment.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
