For those who are low-income and uninsured/underinsured A Sliding Fee Scale discount program is available- Based on the Federal Poverty Guidelines (income level and number of people in the household).

Proof of income must be provided (contact the Center for eligibility details).

Amount of co-payment will be determined based on information provided to the Center and the Sliding Fee Scale

The Center accepts patients from all areas, regardless of their address.

Most patients served, however, reside in the ten (10) county northwest area of Georgia, including Dade, Walker, Catoosa, Chattooga; Jackson and DeKalb Counties in Alabama; and Hamilton and Marion Counties in Tennessee.

Hours:

Medical: Mon. - Fri. 7:30 a.m. - 7:00 p.m.
Sat. 9:00 a.m. - 3:00 p.m.

Pediatric: Mon. 8:30 a.m. - 6:00 p.m.
Tues. - Thurs. 8:00 a.m. - 6:00 p.m.

Lookout Mountain Community Services:

M-F 8:30a.m. - 5:00 p.m. (established patient appointments)
M-F 8:30a.m. -4:00p.m. (new patient walk-ins)
