The Next Step Program is an innovative and unique intensive case management model structured to provide temporary shelter in a service-intensive environment in order to redirect chronically homeless men to permanent housing. Utilizing a multi-disciplinary team approach, this 234 bed facility for men, under contract with the Department of Homeless Services, has a focus of setting goals and monitoring progress for successful transition out of the shelter system within 90 days or less.

Rapid Re-Housing Assistance for client returning from housing placement
Case Management-including assessment and service planning
Benefits and Entitlement Support
Substance Abuse Services
Housing Assistance and Referrals
Housing Readiness Training
Medical Services (Provided in partnership with Citicare)
Discharge Planning & Aftercare

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
