BY APPOINTMENT ONLY - call regarding walk-in availability. Primary services provided include chronic disease management for diabetes, high blood pressure, and high cholesterol; gynecological and family planning services; STD diagnosis and treatment; and medication and lab services. Most services require an appointment; however, limited medical walk-in hours as well as same day appointments are available. For all walk-in clinics, a limited number of patients are seen on a first-come, first-served basis.
The dental clinic offers restorative care as well as emergency extractions. The restorative clinic operates Monday through Friday by appointment only. During this time, we offer full exam and assessments, fillings, cleanings, select root canals, flippers and extractions. We also use digital x-rays and electronic medical records to provide quality care to our patients. For most of these services, a full exam is required before starting treatment.
Evening emergency visits and extractions are offered by appointment only. The frequency of evening clinics is generally based on the availability of volunteer dentists from the community. Evening clinics start at 4:30pm and are group appointments, which means that the order in which patients arrive and sign in is the order in which they will be seen. Extraction clinics are also offered on the second and fourth Saturdays of every month.
We do not offer the following services at this time: partials or dentures, crowns, bridges, implants, braces, teeth whitening, or involved oral surgery (biopsies, bony-impacted extractions, cleft palate surgeries, etc.)

Circle Health Services' mission is to provide quality health care and related services free of charge to those who lack appropriate alternatives, including the uninsured, underinsured and those individuals with coverage through Medicaid or Medicare.
The first step to becoming a patient is to call (216) 721-4010 to schedule a registration appointment. All new patients must complete the registration process prior to their first medical, dental or behavioral health appointment. To register as a Circle Health Services patient, please bring the following documents with you to your appointment:
Valid state ID
Proof of income (pay stubs, tax forms, etc.) - We determine your income by looking at the ways you are supported, even if you are not working.
Insurance card, if relevant
Proof of family size (birth certificates of children, tax forms, etc.)
General medical services
Specialty clinics by appointment only
Routine physicals by appointment only (MUST HAVE PAPERWORK)
Chronic care - hypertension, diabetes, asthma, and high cholesterol
Pregnancy tests for women over age 18
Women's Health Services by appointment only
Education and prevention for HIV & STDs (testing is also available)
Annual exams/pap smears
Family planning services and counseling
- Male & Female STDs, birth control refills (with up-to-date PAP ONLY)
- TB test and lab test results
