It is our mission to provide counseling and comprehensive medical and educational services for the medically underserved.

Services Provided:

    Basic Primary Health Care
    Preventative Health Care Services (flu shots, etc.)
    Diabetes Counseling and Supplies
    Diabetic Eye Exams
    Limited Psychological / Social Work Services
    Medication Assistance to St. Luke's patients (no narcotics are dispensed)

Eligibility:

- Have no medical insurance or are underinsured.

- Are 18 years or older.

- Have not been patients at the Center for Family Health in the past six months.

Hours:

Monday and Wednesday 7:30am-10:30am

Tuesday and Thursday: 5:30pm-8:00pm

Friday, Saturday, and Sunday: Closed
