For those with no health insurance who do not qualify for any other programs or assistance, CHSA offers a sliding fee schedule based on Federal Poverty Level (FPL), family size and income. Our health center staff members can help you decide on the payment plan most appropriate for you.


PAYMENTS ACCEPTED: Private Insurance, Medicaid, CHIP, Medicare (where applicable), Medicaid-on-hold for patients 2 years and under, Cash, and Uninsured assistance available based on family size and income (a sliding fee).
CHSA provides care on a sliding fee basis. The sliding fee schedule is based upon family size and income. Please speak with our staff to see if you are eligible for this service. Thank you.

These numbers above represent 100% of the Federal Poverty Rate. In order to get legal aid from some offices, they use a sliding fee scale. When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.

      They limited on what they can give and are too busy with STD and medicaid/well woman exams to take care of scheduled cash paying patients.
