COVID-19 (Coronavirus) Information for Patients
Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:
Symptoms such as fever, cough or shortness of breath, fatigue, headache, muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose.
Contact with someone with confirmed COVID-19 within 14 days of onset of systems
This clinic operates under a SLIDING SCALE model. This means that it MAY NOT be free depending on your income. You will be required to prove financial need in order to receive free services or services at a reduced cost. This is a health care center funded by the federal government. This means even if you have no insurance you can be covered. The center is also income based for those making an income. This health center can cover services such as checkups, treatment, pregnancy care (where applicable), immunizations and child care (where applicable), prescription medicine and mental and substance abuse where applicable. The Pasadena Health Center offers a sliding scale fee discount for patients who qualify. This reduction is based on the number of dependents residing at the patient's residence, as well as total income earned in the household. If you have a problem making payment for services, discuss this with our staff before services are rendered.
Eligibility Policy for Pasadena Health Center
To be considered eligible for the sliding fee discount you must present the following:
Valid Texas Drivers License or Texas ID (for Adults)
Valid ID= Birth Certificate (for each child)
In addition the following must also be presented:
2 pay check stubs for each person in the same household that is working.
If you do not get paid by check, then a letter from an employer stating the amount of monthly gross earnings (or $ per hour and number of hours worked per week).
Proof of assistance from Texas Unemployment Commission (if receiving checks from the Commission.
Proof of any other financial assistance (child support, food stamps, etc.)
Letter of proof if a dependent in the same household is a full time college student.
Medicaid eligibility letter: this should be the same month the patient is being seen.
Bank Statement - last two (have to be current)
Income Tax - latest one filed
For eligibility renewal - need to present new current documents
