Eligibility Criteria: 55 year and older, Live in the following communities: Avon, Braintree, Boston, Canton, Dedham, Holbrook, Milton, Quincy, Randolph, Norwood, Sharon, and Stoughton & Weymouth, Must have health conditions that would qualify you for nursing facility care determined by the state., Ability to live safely in the community with additional supports
Payment: Medicare, Medicaid and Private Pay.
This Health Center is based on Sliding scale fees which are variable prices for products, services, or taxes based on a customer's ability to pay. Such fees are thereby reduced for those who have lower incomes, or alternatively, less money to spare after their personal expenses, regardless of income.
