All new and existing clients that need eligibility or re-certification will need to go through our new Eligibility process. You will need to have eligibility determined for services PRIOR to making your clinic appointment. If applying for children, a social security card for yourself and each child is required.

Serving children ages 6 months to 20 years of age including:
    Exams
    X-Rays
    Cleanings
    Fluoride
    Fillings
    Extractions
    Sealants
    Pulp treatments
    Stainless steel crowns
    Adults: Emergency Dental Care Only
    Toothaches
    Facial swelling
    Infections
    Pain
