Hackley Community Care Center services include:

• Preventive care (Physicals for all ages, flu shots, screenings)
• Evaluation and treatment of acute and chronic conditions
• Treatment for all ages, from newborn to geriatrics
• Minor office surgeries and procedures
• Hearing and vision testing
• On-site lab

Hackley Community Care offers a sliding scale based on your family size and income and offers reduced fees to eligible patients. Patients will not be turned away due to the inability to pay.

Medical Hours:

7:40am 8:00pm, Monday
7:40am   5:00pm, Tuesday
8:00am   7:00pm, Wednesday
8:00am 5:00pm, Thursday
7:40am   5:00pm, Friday
