Hackley Community Care (HCC) participates with most insurance companies. Please call 231-733-6750 for more information.
Hackley Community Care staff provides assistance with the application and enrollment process in the Health Insurance Marketplace, Medicaid Programs and Sliding Fee Scale. All programs are based on your income and require that you bring in proof of income or tax return, along with a picture ID. If you do not have an income or do not file taxes you will need to explain how you support yourself. Patients will not be turned away due to the inability to pay.
Sliding Fee Scale
This program (not an insurance plan) is based on your family size and income and offers reduced fees to eligible HCC patients. Please be ready to present your picture ID and Sliding Fee Scale card at every visit.
We ask that you be prepared to pay any co payments or deductibles at the time of service. We will send you an itemized statement for any fees not covered by your insurance. Payment arrangements may also be made with our billing staff. Call 231-733-6607 for assistance.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income. When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:
- Symptoms such as fever, cough or shortness of breath, fatigue, headache
- Symptoms such as muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose.
- Contact with someone with confirmed COVID-19 within 14 days of onset of systems
Mild Covid Symptoms
- Low-grade fever (approx 100 degrees Fahrenheit for adults)
- Nasal congestion
- Runny nose
- Mild, dry cough
- Mild body aches
Moderate Covid Symptoms
- Fever above 100.4 F
- Persistent cough
- Temporary shortness of breath when you exert yourself
- Exhaustion, need to stay in bed
Severe Covid Symptoms
- Constant trouble breathing
- Persistent chest pain or pressure
- Trouble staying awake
- Blue lips or face

Uninsured, having a hard time getting assistance from Ottawa county. Husband works in construction making $400-$600 a week. No pay stubs but can provide information from boss. I have a medical dr who takes care of my diabetes, looking for a psychiatrist for my bipolar, just found out i have dental w my free advantahe humana plan. 1 cleaning a year, 1 bite wing a year and any anethesia needed. My monthly income is 1418. I have no script coverage and have several different meds i take. I have been buying them through a discount place but cannot use that cost on my tax.

I have been to this clinic.I needed medical care. This clinic was not free. I paid full price. The staff was unfriendly. I waited several hours They made me wait for OVER 2 hours in an exam room. I could hear her (Dr. NAME WITHELD) socializing in the hallway the entire time. I felt extremely disrespected. She then came in the room and proceeded to treat me like a criminal. After 5 minutes of this I told her I would not except her as my physician and left. She then proceeded to add derogatory and damaging things to my medical record. I was forced to hire an attorney to sue her to remove these defamatory comments. I have no doubt that she did this as an act of vengeance to hurt me because I denied her care. I highly recommend NOT using this facility for ANY reason. In my opinion They are reckless, disrespectful and extremely unprofessional. I was under the impression that doctors take an oath to DO NO HARM. I suspect They missed that day of their education. The absolute worst experience I have ever had with a medical care provider.
