Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:
Symptoms such as fever, cough or shortness of breath, fatigue, headache, muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose.
Contact with someone with confirmed COVID-19 within 14 days of onset of systems

Thursday 8:30 am 5:30 pm
Friday 8:30 am 5:30 pm
Saturday 8:30 am 12:30 pm*
*Open the first and third Saturdays of the month.

You can come to Erie HealthReach Waukegan for everything from free pregnancy tests, prenatal care and school physicals to cholesterol checks, diabetes management and dental check-ups for your children. The bilingual staff speaks Spanish and can connect you to interpreters speaking other languages. They partner with Northwestern Lake Forest Hospital, Advocate Condell Medical Center and Vista Health System.
