Center Street Community Health Center, a community-based and patient-directed healthcare facility that provides accessible, high-quality medical, dental and behavioral health services for all ages. We look forward to serving you with your healthcare needs.

We are a Federally Qualified Health Center (FQHC) that will accept uninsured individuals as well as Medicaid, Medicare and Insurance. If you do not have health insurance we offer a sliding fee scale which is based on family size and income. We also have staff members that can assist you with enrolling in health insurance or Medicaid if you qualify.
