COVID-19 (Coronavirus) Information for Patients
Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:
Symptoms such as fever, cough or shortness of breath, fatigue, headache, muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose.
Contact with someone with confirmed COVID-19 within 14 days of onset of systems

New applicant must be a resident of Spartanburg County and have no medical coverage of any kind.
APPLICATIONS ARE TAKEN IN THE LOBBY:
Mondays - 10:00 am to 1:00 pm
Thursdays - 1:00 pm to 4:00 pm
PLEASE BRING THE FOLLOWING:
Â· Picture I.D. with current address
Â· Social Security Card
Â· Proof of income for all individuals in household for the past 4 weeks
Â· If no income bring a statement from whomever is assisting/helping you get by
Â· Food Stamp letter (if applicable)
Â· Disability/Social Security/SSI award letter (if applicable)
